Are You Eligible?
If you are a veteran or war widow/widower holding a DVA Gold or White Card, you may be eligible for government-funded community nursing services.
An assessment is required to determine your clinical and/or personal care needs.
To access services, you will need a referral from one of the following:

Getting Started
Once a referral is received, we will:
